Medical Records Policy
To ensure timely and accurate independent medical evaluations (IMEs) and other report, all medical records must be submitted according to the following guidelines:
- You may send up to 200 pages by email: PPGMedicalRecords@gmail.com
- Records must be received at least 72 business hours before the appointment
- Records must be sorted and organized. Unsorted files will incur an additional preparation fee
- Hard copies may be mailed or shipped to our administrative office:

Late Medical Records
- If medical records are not received on time, the physician will document that they were unavailable in the IME/record review report
- Late submissions may delay dictation until the doctor’s next scheduled review day
- Medical records received less than 48 hours before the evaluation will require an addendum review fee
Following these requirements helps us provide accurate, defensible, and timely IME reports.
